2024 - 2025 RANK ONE ONLINE FORMS ARE AVAILABLE

ALL Students Participating in MCISD Athletics will Need to Complete a New Physical This Year

New Athletic Physical Exams will begin to be accepted AFTER April 1, 2024 for the upcoming 2024-2025 school year.

All MCISD student athletes must have all MCISD / UIL online forms completed on Rank One Sport and a current physical on file with the Athletic Trainers PRIOR to any participation in any practice, scrimmage, performance, or contest before, during, or after school. This includes MCISD athletic periods.

Only the UIL Physical (PPE) forms with 2024 printed on the upper Right hand corner of the UIL Medical History (MH) will be accepted (back side of the physical exam). Links to the PPE and MH forms are below for both English and Spanish versions of the forms.

The UIL / MCISD PPE and MH may be filled in online by the parent / guardian & the student-athlete, printed, and then taken the Medical Doctor (MD) for completion. Please make sure the student-athlete fills in their School Name and their School ID # and make sure the MD includes their printed name, address, date, and their signature. Also do not forget the signatures at the bottom of the MH page. Please turn in your completed PPE & MH paper copy to the MHS Athletic Trainers, Lori Armendariz and Joaquin Rios, or your MHS Athletic Coach.

*If submitting physicals through email, NO PICTURES of the Physical Exam document will be accepted. Only submit SCANNED copies.

Mission CISD is utilizing an online version of the MCISD / UIL Athletic Signature forms in conjunction with Rank One Sport. Please complete the online version of the Mission CISD / UIL Athletic Signature forms by following the Red link below.

The MCISD Athletic Trainers will be notified if you answer "Yes" to any of the questions on the UIL Medical History. Any "Yes" answers to questions on the UIL Medical History form will require further medical review by the MCISD Athletic Trainers and also may require a physical examination & written clearance from a physician, physician assistant, chiropractor, or nurse practitioner.

The 2023 - 2024 MCISD Physical packet will expire on July 31, 2024 for MCISD Athletic purposes only.
